  • Government Issued ID
  • Reference Number/Control Number

Process:

  1. Sign-in Logbook.

In case of Death of Livestock due to disease/calamity damage

Process:

  1. Report to the office and sign-in logbook.
  2. Submit documents (police blotter, barangay certification & picture of the dead animal).

In case of Crop loss due to pest and disease/calamity damage

Process:

  1. Report to office and sign-in logbook.
  2. Submit Claim of Indemnity and barangay certification.

In case Claim for Indemnity/Notice of loss is Approved

Process:

  1. Receive check.
