Requirements:
- Official Receipt
- LTO Certificate of Registration with Latest OR (Photocopy)
- Mayor’s Permit
- Police Clearance
- Professional License of Driver
- Police Inspection Certificate
Process:
- After the issuance of Mayor’s Permit proceed at the SB Office, obtain and fill-up Application Form.
- Produce/present documents needed.
- Wait for the preparation of the MTOP.
- Wait for the approval and signing of MTOP.
- Client receives the MTOP.
