Requirements:
- Certificate of Live Birth containing the wrong entry from LCRO and PSA Copy
- Certificate of Baptism
- Voter’s Registration Record
- School Record (Form 137, diploma)
- Marriage Certificate
- Birth Certificate of Children
- Any order a document that proves the first name such as old ID’s, Driver’s License, SSS, Insurance, Passbooks, Service Records, Appointments, etc.
- Police Clearance
- NBI Clearance
- Employer’s Clearance (if Employed)
- Affidavit of Non-employment (Not Employed)
Process:
- Fill-up Petition Form and submit requirements in two copies each.
- Pay the required fees.
- Present proof of payment.
- Send the documents to the PSA, Manila through LBC for affirmation.
- Receive and sign in the logbook.
