Registration of Birth Certificate

Requirement:

  • Accomplished Mun. Form 102 (Certificate of Live Birth) from Hospital Authority and Rural Health Unit

Process:

  1. RHM/Hospital Records Officer submits Mun. Form 102.
  2. Wait for the recording of the document of your copy and log in Logbook.
